Here is a sample of a performance evaluation system to give you an idea of what one might behold like. This is in fact the actual performance evaluation I have used in my accounting practice. Many of the questions might seem to you to be geared towards professional employees, but we have the same standards for our receptionist. Remember that your performance evaluation system should be tailored to your company, your employees and your business plan.

 3-4 Not a strength. This individual meets some behavior and skills expectations in this situation, but sometimes falls short
